New Patient Registration
We are happy to register patients who live within our practice area. We serve the villages of Anstey, Cropston, Rothley (Swithland Lane & Westfield Road only), Thurcaston, Newtown Linford (upto Lee John Woods entrance) and Swithland with a population of approximatley 7300 patients.

You can either collect a form from the surgery or download the relevant form below.
Child Registration Form under 16 years Jan 2019
Adult Registration Form -16 years and over Jan 2019
Please bring these into surgery along with two forms of identification, a photographic form of ID (such as a PASSPORT or DRIVING LICENSE) and proof of your home address (such as a recent BANK STATEMENT or UTILITY BILL). Each adult registering with the surgery will need to attend for their application to be processed.
As a new contractual agreement, we are now obliged to inform all new patients registering with Anstey Surgery of your allocated named accountable GP.
If your application is successful your allocated GP will be DR NW Osborne
This is purely an administrative procedure. We have an open policy here for patients; you may make an appointment to see whichever GP you wish.
In addition, if you are aged 75 or over you will also be notified, in writing, of a named GP who will have overall responsibility for the care and support that our surgery provides to you.
We are able to treat friends and relatives who are staying with you, please ring the surgery for advice.
